The answer is many and varied.
There is not even close to a single way, or even a best or easiest or fastest
way, it depends massively on who is making the site, what machines it will
run on, and such things.
Google, for example, uses in excess of 100,000 computers to deliver it's
services these days (ever wondered why it is so fast?) and uses it's own
customised version of linux to run on.

The majority of 'medium' sites are either running on custom 'web site' systems
(php, zope, and many commercial relatives), or are developed in perl, python,
or java, with c# starting to show up.
